90000 Adductor Longus Sprains and Tears (Athletic Pubalgia) 90001 90002 90003 Hernia and general surgeons will sometimes see these patients, who think maybe they have a hernia or a «sports hernia». In most cases, patients should absolutely avoid surgery as initial therapy, and instead, these patients should be first referred to a specialist or a rehab program. 90004 90005 90002 The hip adductors are a powerful muscle group. They consist of the adductor magnus, minimus, brevis, and longus. The gracilis and pectineus muscles also are included. All of the adductor muscles are innervated by the obturator nerve (L2-L4) except the pectineus, which is innervated by the femoral nerve (L2-L4). The adductor magnus also is innervated by the tibial nerve (L4-S3). 90005 90002 The origins and insertions of each of the hip adductors are noted below: 90005 90010 90011 Adductor magnus / minimus: origin is the inferior pubic ramus / ischial tuberosity; insertion, linea aspera / adductor tubercle 90012 90011 Adductor brevis: origin is the inferior pubic ramus; insertion, linea aspera / pectineal line 90012 90011 Adductor longus: origin is the anterior pubic ramus; insertion, linea aspera 90012 90011 Gracilis: origin is the inferior symphysis / pubic arch; insertion, proximal medial tibia 90012 90011 Pectineus: origin is the pectineal line of pubis; insertion, pectineal line of femur 90012 90021 90002 90003 Adductor muscle strain 90004 involves inflammation and pain along the inner thigh muscles and groin. These muscles allow the hip and leg to move across the body. Muscle attaches to pubic bone via tendon. The injury occurs at this junction between the muscle and tendon or at the tendon attachment to the pelvic bone. The strain may be a partial or complete tear of any or all of the adductor muscles. 90005 90002 90003 An MRI 90004 will pick up adductor tears and sprains, which may also be associated with osteitis pubalgia, or even other simultaneous injuries. 90005 90002 These injuries can be graded.A 90003 grade I strain 90004 is a mild strain. There is a slight pull without obvious tearing (that is, the tearing is microscopic). There is no loss of strength, and the muscle and tendon are the correct length. A 90003 grade 2 strain 90004 is a moderate strain. There is tearing of fibers within the substance of the tendon, at the bone-to-tendon junction, or at the muscle-tendon junction. The length of the tendon or whole muscle-tendon-bone unit is increased, and there is usually decreased strength.A 90003 grade 3 strain 90004 is a complete rupture of the tendon off of the pubic bone. 90005 90002 Results from one study on adductor longus tears in NFL players are shown here: 90003 Fourteen NFL ™ players were treated nonoperatively, and 5 players were treated with surgical repair 90004 using suture anchors. In both groups, all players eventually returned to play in the National Football League. 90003 Mean time for return to play was 6.1 ± 3.1 weeks (range, 3-12 weeks) for the nonoperative group 90004 and 12.0 ± 2.5 weeks (range, 10-16 weeks) for the operative group (P = .001). One player in the operative group suffered the complication of a draining wound and heterotopic ossification. Players represented a variety of positions, and 12 of 19 (63%) had experienced prior symptoms or events. (Reference = 90003 Theodore F. Schlegel, etal. The American Journal of Sports Medicine, Vol. 37, No. 7 2009). 90004 Nonoperative management includes NSAIDS with stretching and massage therapy, and a 3-6 week gradually progressive rehab program.Read more: http://www.harbinclinic.com/umages/pdfs/adductor.pdf 90005 90002 Watch video about adductor longus strain nonoperative management at http://www.youtube.com/watch?v=cDuzGs5_Pbk 90005 90048 General Management Algorithm: 90049 90002 If you have this injury, it is best to be 90003 referred to a physical therapist or rehab program 90004 who can design a gradual 3 — 8 week program for you. These programs will involve slow, but gradually more strenuous stretching and strengthening exercises designed specifically for you and your injury.90005 90002 Rest from the insulting activity should last for at least 3-8 weeks depending on the severity of your injury, with 90003 gradual increase in activity 90004 as guided by therapist, rehab physician, or your care provider. 90003 Nonsteroidal anti-inflammatory medications 90004, such as aspirin, Celebrex, and ibuprofen can be used with or without other minor pain relievers, such as acetaminophen. Take these as directed by your physician. 90003 Ice packs 90004 reduce inflammation for acute and chronic cases and are also useful. They can be applied every 2 to 3 hours for inflammation and pain and immediately after any activity that aggravates your symptoms. 90003 Heat 90004 may also be used 90003 before 90004 performing stretching activities. 90005 90002 Surgery is rarely necessary to re-attach tendons if it pulls off the pelvic bone. Surgery may be recommended if chronic, persistent pain exists at the groin for more than 3 months despite appropriate conservative treatment for strains, but this is rare. 90003 Unfortunately, suturing, or sewing, torn muscle is usually not successful.Likewise, usual hernia repairs for this injury do not often help. 90004 90005 90002 For reference on above information and to read more about management: 90005 90002 http://emedicine.medscape.com/article/307308 90005 90002 Feel free to email us for an appointment or to aid in your diagnosis at 90003 nychernia @ gmail.com 90004 90005 90002 90048 90080 Related 90081 90049 90005.90000 A Rare Case of Adductor Longus Muscle Rupture 90001 90002 An adductor longus muscle rupture is a rare injury. This case report describes a 32-year-old patient with an adductor longus rupture. The trauma mechanism was a hyperabduction movement during a soccer game. Nonoperative treatment was initiated. After a follow-up of 4 years, the patient was without pain but a small swelling was still visible. This report describes the anatomy, pathophysiology, and evidence-based treatment of adductor longus rupture.90003 90004 1. Introduction 90005 90002 The adductor longus muscle is a triangular shaped long and relatively thin muscle. The muscle arises from the ramus superior of the pubic bone and is inserted onto the middle part of the linea aspera. The muscle fibres are narrow proximally and straggle more distally (Figure 1). The anterior branch of the obturator nerve innervates the muscle. It is involved in adduction, external rotation, and anteversion of the hip joint. 90003 90008 90009 90002 Subtotal or total ruptures of the adductor longus muscle are rare [1-7].This injury should be included in the differential diagnosis in patients with chronic groin pain or previous injuries. An important finding is a history of trauma before the onset of pain in the groin region. A common physical finding is the presence of a swelling on the inner side of the thigh. 90003 90002 Due to the low incidence of this injury reports on the treatment are scarce in the literature [1-7]. We pursued to describe an uncommon case and summarize the available literature. 90003 90004 2.Case Report 90005 90002 A 32-year-old man presented in our outpatient clinic with pain in the right groin and proximal thigh, which had been present for three months. The symptoms had started during a soccer game. After kicking the ball with his right foot, he felt an acute pain in the groin and proximal thigh. At the same time he experienced some sort of «popping / cracking sensation» in the upper thigh. Following the injury, he was unable to continue playing. He had no history of groin pain or adductor problems before this injury.Intensive physiotherapy after the incident gave no relief of his complaints. When the patient presented to our clinic he still complained of pain and weakness of the right leg. 90003 90002 At the physical examination there was no swelling or hematoma. Range of motion of both hip joints was normal and without pain. During contraction of the adductors against resistance there was a palpable mass at the inner proximal thigh with a dimple being more distal. Testing of the hamstrings showed no abnormalities.90003 90002 Conventional radiographs of the pelvis and the right hip showed no osseous abnormalities. Magnetic resonance imaging (MRI) revealed an intramuscular hyperintense region in the right adductor longus, with a craniocaudal length of 4.2 centimetres. There was no retraction of the muscle. On the basis of the clinical findings and MRI examination the diagnosis was made: an intramuscular adductor longus rupture (Figure 2). 90003 90002 Conservative treatment was started, consisting of a physiotherapy focused on stretching of the muscle followed by strength training. After eight weeks, the complaints had resolved and the patient had resumed playing soccer. 90003 90002 Four years later, the patient presented with a persistent swelling on the inner side of his thigh without pain. 90003 90002 Additional MRI examination showed subtle higher signal on T1-weighted images in the previous adductor longus rupture, without any structural abnormalities (Figure 3). The findings were discussed with the patient and no further treatment was initiated. 90003 90004 3. Discussion 90005 90002 In this paper we describe an uncommon case of a patient with a rupture of the adductor longus muscle, which is a rare injury [2, 3].90003 90002 One should consider an adductor longus tear in patients with pain in the groin region [1, 3-5]. An important finding of rupture is the presence of a trauma prior to the onset of complaints. The trauma mechanism can give a clue to the diagnosis. A typical trauma mechanism is an eccentric overload caused by forced abduction during contraction of the adductor muscle group [2]. Combinations with extension and endorotation have also been described [3]. Besides the pain, a hematoma and weakness of the adductors can be present. In cases with a tumour-like swelling in the proximal medial part of the thigh, an old rupture of the adductor longus muscle should be considered [1]. An important differential diagnosis is a strangulated inguinal hernia. This usually causes acute pain in the groin region [6]. An important difference in such case is the absence of a trauma. 90003 90002 Little is known about the treatment of an isolated rupture of the adductor longus. Schlegel et al. treated 19 national football league patients, of whom 14 underwent conservative treatment and 5 underwent operative one [4].All players had a rupture of the adductor longus on the proximal side. Twelve (63%) had experienced some form of antec 90003.90000 musculus adductor longus — це … Що таке musculus adductor longus? 90001 90002 90003 90004 Musculus adductor longus 90005 — Adduktorengruppe, der Musculus adductor longus bildet das mittlere Drittel Ursprung Schambein (Ramus superior) Ansatz O … Deutsch Wikipedia 90006 90007 90002 90003 90004 Musculus adductor longus 90005 — ilgasis pritraukiamasis raumuo statusas T sritis gyvūnų anatomija, gyvūnų morfologija atitikmenys: lot.
